Make a
plan
to stay healthy.
Work with a Live Well At Home coach (create link to service description) for specialized help with this risk factor.
Avoid falls that cause injury.
Learn as much as you can about your chronic condition from your doctor and clinic, by
enrolling in a class
, or through other community resources.
Talk with others who have a similar condition and learn how they cope with the condition.
Work with your doctor to use the best treatment plan for you. Try new therapies and treatments to feel better even if you are feeling “blue” or “down in the dumps”.
Use a family member or friend to help you stay healthy and deal with health issues. Ask for help, if you need it.
Keep a diary of your progress and what works best for you.
