Looking for more help? There are many places to get more information about services to help you live at home and pay for them.

Live Well at Home Providers offer professional, more intense, on-going services to help you manage your risk factors and buy the help that you need. They can help you learn more about your risk factors, best ways to manage them, and good ideas on how to keep them under control. These providers include:

Duluth Area: Arrowhead Area Agency on Aging,

Mankato Area: Mankato Area Living At Home Block Nurse Program, VINE Faith in Action, Aging Services for Communities

St. Cloud Area: Great River Faith in Action, East Central Senior Resource Center. The average cost is about $50 per hour.

Caregiver Support Providers are professionals trained in family caregiving and aging issues, They will work with you – the family caregiver – to learn your personal risks with caring for a family member, offer you ideas on how to cope and stay healthy, deal with complex family issues, find help in your area, and help you set and reach goals. They will also teach you ways to help you manage your family member’s risk factors. For a provider go to www.minnesotahelp.info for a listing of caregiver consultants.

Senior LinkAge Line® is a no-cost telephone information and assistance service, which makes it easy for older adults and their families to find local services and community help. You can talk with a trained counselor to learn what kind of service might be helpful, learn ways to pay for services, and connect to public program eligibility. Call the Senior LinkAge Line® at 1-800-333-2433 or email them at senior.linkage@state.mn.us.

Long-term Care Consultation services provided by county staff that connect older adults and persons with disabilities with long-term care needs to various kinds of support. This includes , conducting nursing home pre-admission screening, evaluation for public program eligibility, and helping nursing home residents move back to the community. Their services help people make decisions about long-term care.

The Alzheimer’s Association has information regarding Alzheimer’s disease and related dementias, including: warning signs, an overview of the disease, risks, diagnosis, treatments, and research. There are links to resources available to people living with dementia and their caregivers such as: support groups; care consultation; family, community and professional educational opportunities; multilingual information; and publications.

MinnesotaHelp.info® is a large online database of service providers and resources that help older adults live in the community that includes listings such as: fiscal support entities, support planners, caregiver consultants, meals, rides, yard and home help, help with prescription drugs, and help for Veterans.

The Office of Ombudsman for Long-Term Care provide information and consultation about consumer rights and the regulations that apply to home care services, home and community-based settings, and long-term care facilities.

Fiscal Support Entities (FSEs) can help you hire and employ workers so you get the help that you need to live at home. FSE services help you learn about self-directed supports including, pay for workers, equipment, and supplies; manage your spending; and, follow applicable federal and state laws.

Consumer Directions, Inc. is working with private pay consumers on this project. Depending on what they do for you their charges run about $30-185 per month. To find a provider certified by the Minnesota Department of Human Services go to www.minnesotahelp.info.

Support Planners – General can help you create a plan and budget for buying in-home services. They can help you employ workers, identify ways to make living at home easier and safer for you, and connect you with other supports to live at home. The cost is about $45 per hour.
