About the project

Live Well At Home was developed in 2007-2009 by the Minnesota Board on Aging with grant funding from the Administration on Aging.  It was designed to help older persons and their family caregivers deal directly with issues likely to make living at home more difficult.     

The University of Minnesota determined the areas of risk most likely to cause older adults to move permanently from home.  A study to evaluate and validate the Live Well at Home Rapid Screen© tool was done.  Other key partners in the project were the Arrowhead Area Agency on Aging, Duluth; the Central Minnesota Council on Aging, St. Cloud; and the Minnesota River Area Agency on Aging, Mankato.  The partners pilot-tested tools and service components in each of their regions.   

The Minnesota Board on Aging was awarded a Community Living Program grant from the Administration on Aging for 2009-2011 to continue the Live Well At Home Program.  This will help more older adults and family caregivers across Minnesota continue to live well at home.
